Over 75s GP

Are you age 75 or over? We are required to ensure that there is a named, accountable GP assigned to each patient aged 75 and over. This will be the GP that you have been registered under (not necessarily the GP you usually see).The named accountable GP will take lead responsibility for ensuring that all appropriate services required under the contract are delivered to each of their patients aged 75 and over where required (based on the clinical judgement of the named accountable GP).They will:· work with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient· ensure that the physical and psychological needs of the patient are recognised and responded to by the relevant clinician in the practice· ensure the patient aged 75 years and over has access to a health check. This does not prevent you from seeing any clinician in the practice as you currently do

Home Visits

It is clinically better for you to be seen at the surgery where we have better facilities, diagnostic aids and lighting to assess you. Home visits are for housebound patients and for those too ill to attend the surgery. Home visits are not for those who do not have transport. If you need a home visit, please submit a request via our online triage link. Alternatively, please call the surgery before 9:00amAll home visit requests will be reviewed by the Duty Doctor, who will assess the need for a visit and book an appointment with the Home Visiting Service if appropriate. Once an appointment is booked, the patient will be informed of the time and details. Home visits will be carried out by the GP Hub team

Patient’s Rights

You have the right to access the services we provide according to your clinical need. You can view your medical records if you are registered with the NHS App. You have the right to express a preference to receive services from particular practitioner either generally or in relation to any particular condition, unless there is a reasonable ground for refusing to provide service to the patient or does not routinely perform the service in question within the practice. You have the right to expect a standard of care that would reasonably be provided by one of the Doctors or Practice Nurses

Patients not seen within 3 years

Any registered patient who is between the age of 16 and 75 years who requests a GP appointment; and has not had an appointment with the practice within the last three years will be booked an appointment as soon as is practical
